Description
Fruson is a combination drug containing a short-acting loop diuretic, Frusemide and a long-acting aldosterone antagonist, potassium-sparing diuretic Spironolactone. Spironolactone and Frusemide have different but complementary mechanisms and sites of action. Combination exhibits synergistic diuretic effect. Frusemide inhibits Na+/K+/2Cl- co-transport at ascending limb of loop of Henle and there occurs inhibition of electrolyte and water reabsorption. Spironolactone acts on collecting tubule to block the aldosterone receptors competitively, inhibiting sodium retention and potassium secretion. As a result sodium excretion is largely enhanced and loss of potassium is suppressed and a total positive effect on diuresis is achieved.
Composition
Fruson 20 : Each tablet contains Frusemide BP 20 mg & Spironolactone BP 50 mg.
Indications And Uses
• Essential hypertension • Congestive cardiac failure • Liver cirrhosis, with ascites • Oedema • Resistant oedema associated with secondary hyperaldosteronism • Hyperaldosteronism
Dosage And Administration
1 to 4 tablets daily (50 to 200 mg of Spironolactone and 20 to 80 mg of Frusemide) according to the patient's response.
Side Effect
Generally, this combination preparation is well tolerated. However, a few side effects like nausea, malaise or gastric upset, headache, hypotension, skin rash may be seen which disappears after withdrawal of the drug.
Precaution
Diabetes, Prostatic hypertrophy, Kidney failure, Liver disease, Hypotension, Low volume of circulating blood (hypovolaemia)
Use In Pregnancy Lactation
Fruson 20 should be used in pregnancy only if the expected benefit to the mother is greater than the possible risks to the fetus. As Fruson® 20 passes into breast milk it is recommended that mothers should avoid using this medicine while breastfeeding.
Drug Interactions
Precaution should be taken while co-administration with the following drugs: ACE inhibitors, cardiac glycosides, carbenoxolone, corticosteroids & NSAIDs.
Overdosage
Symptoms of overdosage include drowsiness, dizziness, mental confusion, diarrhea, vomiting, etc. due to excessive diuresis. Corrective measures should be taken to replenish the fluid loss.
Contraindications
Allergy to Furosemide / Spironolactone, Children, Hyperkalaemia, Addison's disease, Renal failure, Anuria, Hyponatraemia.
